Public health emergencies linked to extreme climate events—such as floods and droughts—have intensified, revealing the intricate relationship between climate change and structural inequalities. For traditional populations in rural areas, waters, forests, and Indigenous peoples, the impacts of climate change exacerbate existing socio-environmental vulnerabilities. In Brazil, experiences termed "popular health surveillance" (PHS) have gained relevance by positioning communities as protagonists in defense of life, combining local knowledge with varying levels of participation from the State and academia. In this context, the development of tools, strategies, and actions for disaster mitigation, prevention, and resilience—articulating multiple actors, with an emphasis on engaging populations in risk areas—becomes one of the central challenges for health systems. PHS is fundamental to strengthening communities' adaptive capacity, integrating diverse knowledge, and advocating for more inclusive and democratic public policies.
